Registration

Please complete this form (one for each child) to register for the VBS event at First UMC Sunday, July 18 - July 22, 2010, 6:30 to 8:30 p.m. You do not have to specify all the information for each child (address, phone numbers, etc.). Cost for children is $10.00 5th and 6th graders, $15.00.

For children in 5th or 6th grade we need your permission to allow the children to walk to McDonald's, Dairy Queen, and Dunkin Donuts with the First United Methodist VBS program. To give permission, please check the appropriate box on the form.

Please note: In consideration of our volunteers we ask that you not register your 3 year old if they are not toilet trained or if they will suffer from separation anxiety. Thank you!
